Sure, you can do both of those things. The Costco quilts are sewn through (no baffles) and don't have a lot of down in them, so for more warmth and better shaping a lot of folks buy 2 of them and sew "faux baffles" in both to improve the down loft, then sew the 2 quilts together. There are several DIY theads here on HF that explain how to do this, with lots of pix - just search for "Costco UQ" and "faux baffles"
For more warmth you can indeed put a middle layer of fleece or other insulation of your choice in between the 2 quilts, or you could add more down in the faux-baffle channels (maybe using repurposed down from a sleeping bag or jacket). If you sew a fleece blanket between the quilts it will (obviously) make your UQ considerably heavier and bulky to pack, but it's a lot easier than adding more down in the faux baffles!
Sewing ripstop to the outside bottom of your UQ is good protection and also improves body heat retention & wind blocking.
