Doesn't affect my lay, though it does seem to affect a few people. I personally think it's an overactive imagination on the part of some. They feel the UQ, and then focus on that obsessively, imagining some sort of pressing on their heels and all sorts of stuff. I'm just not that obsessive.
Sometimes it's hard to tell the difference between what you are imagining and what is real. Last year I went out with my beagle Joey to a group hang, and the first night, the temps were in the low teens. Beagle Joey and I almost froze to death. Next night, I had someone check my UQ suspension (hard to do by yourself) and he found it was sagging. He tightened it up, and though the next night was colder, beagle Joey and I slept like babies.

The only thing an underquilt does is keep you warm in the most comfortable way. Other than that, it has no affect on how you use your hammock.

I'm not a very experienced hammock camper but when I tried an underquilt for the first time (last weekend) noticed right away it was a little tighter with the UQ on. It wasn't much but noticeable and I must add that I didn't do much tweaking of the UQ suspension to address it as I had no problems getting comfortable.
I can still get a good diagonal lay with my UQ. t=The hammock is slightly more restrictive than without the UQ, but not enough to affect my overall comfort. If you use a 3/4 length quilt like me, I would advise using a pillow- which helps keep the UQ suspension from pressing in on your head.

If you fuss with it, you can probably find the "just right" amount of tension with the bungee suspension lines. But this is difficult because you can't do it while in the hammock and if you have someone else in the hammock while you make adjustments, that person isn't you. That's one of the reasons I use a UQP (under quilt protector). because covers the UQ ends, it means I don't have to be so critical with the UQ adjustments.
In order to see what few have seen, you must go where few have gone. And DO what few have done.
Generally with a gathered end hammock the lay isn't affected but the width of the UQ is important - hammock gear specifically has more narrow uq's and ugq uq's tend to be wider - I am a side sleeper and hence "push" the sides out as I sleep. I am preferential to Underground Quilts as they are nice and wide and at least for me provide full lateral insulation where the Hammock Gear UQ usually gave me issues somewhere in the night and I would wake up and have to fiddle with things to get warm again. It may be my imagination or the fact that UGQ are all mostly custom work, but I felt the suspension for my Zeppelin was beefier and more robust which held it in place better.
