Sorry to resurrect this, but I just bought materials to make a CS 6.0 UQ and wanted to make sure I understood all of the initial dimensions correctly. Do you also include a 1/2" seam allowance on the liner and channels?
I found a what I think is a pretty good video by Sailrite on making a box cushion and I think the techniques are similar, but the starting dimensions for the fabric are different. I believe they cut the fabric smaller so the cushion is held tightly by the fabric--obviously not something you want for an underquilt.

Yes, you need seam allowances on pretty much everything since they will be sewn into the "sandwich".
